Payment & Insurance Information

Types of Payment Accepted
You may pay with accepted health insurance, a personal check, cash, Visa, MasterCard, American Express or Discover.   Payment for your co-pay and deductible amounts is required by your insurance company at the time of service or may be considered as fraud.

Initial Point Family Medicine accepts most health insurance. We recommend to call your insurance company to be sure Initial Point is an in-network provider with your plan.

We accept Workman’s Compensation Insurance through the State of Idaho and are able to perform and complete DOT drivers’ physicals.

Idaho Workman’s Compensation insurance is accepted and we provide Department of Transportation physicals.  Please note that school/sports/camp physicals are generally not covered by insurances benefits.

Medicare & Medicaid
We are taking new Medicaid patients.  Although Initial Point has chosen not to continue as a Medicare provider, you may continue to come to us for medical, health and wellness services through an Assured direct care membership.

New Patients
For your first visit, please be ready to pay in full at the time of service.

Your Responsibility
For your protection and also for your insurance, it is required to have a current state-issued driver’s license or personal identification card with you at the time of service. Your current insurance information and current billing information is also required.  If you do not bring your insurance information to an appointment, it may be re-scheduled.

No Health Insurance?
Initial Point provides the Assured direct care membership option.  It combines the most-often-used primary care services into an affordable flat-monthly-fee package for comprehensive and personalized healthcare and a higher level of convenience.

If you have no insurance and you are paying cash, please ask us about the  cost of cash-only visits before your appointment. Payment plans are also available.

We will work with you in good faith to provide medical care while resolving any billing issues. If there is non-cooperation with the arrangements, please be aware that outstanding bills may be sent to a collection agency.
