A complete medical history is necessary to allow your Initial Point provider to assess your needs, make diagnoses and provide treatment recommendations. Please fully complete the forms and verify the accuracy of the information. This will help your provider to better know your personal medical history, individual needs, medications, and allergies.

There may be additional forms to complete when you have scheduled an appointment. You may need to come early for your appointment to complete them. Please bring your ID and any insurance information to each appointment (medical, workman’s compensation, or from a motor vehicle accident).

In addition, please bring a list of your medications and the current dosages. If you do not bring this information or are late for your appointment, we reserve the right to reschedule your appointment.  We appreciate your care in completing this advance preparation for your appointment.

Planning for Health Care Expenses

Understanding the price of your medical care can make it easier for you to plan for healthcare expenses. For your convenience, we offer price estimates by calling the office for some of our most common medical services. Estimates are not a guarantee of final charges; your actual charges may vary depending on your condition, unknown circumstances or complications, medication, equipment and supplies used by your provider, the actual treatment provided, the length of time spent in both surgery and recovery, and other variables. Your ultimate financial responsibility will be dependent on your insurance plan or yourself if you have no insurance.

Estimates for services being performed at Initial Point Family Medicine can be requested at (208) 884-0835, Monday-Friday, 8 a.m. to 5 p.m., MST. We will need the appropriate CPT code to assist you, which you can obtain from your provider.
